How to Add a Drawable Field to a Form

If you are more of a visual person, like myself, it can be difficult to try and describe where I am experiencing pain or unusual functionality via text. With the Drawable Field, you can upload a picture or diagram, that patients can mark or notate on, directly from their phone!

Drawable Settings

  1. How to name your drawable image:

  • Field - To create a new Field, select -new field- in the dropdown.

  • Label - is what is displayed on the form for the patients.

  • Name - is an internal value used to identify the field for use on things like replacement strings.

2. How to add an image:

Click on Choose Image.

  • Click the +Add Image button at the top right of the Image page.

  • Select the image you want to display from your computer.

  • The image will appear as a thumbnail on the Image page.

  • Click on the image you want to display.

3. Drawable Field placement:

  1. Label Position: Here you can select for your label to appear on the Top of the Drawable Image or to the Left of the image.

  2. Alignment: Here you can adjust the alignment of the Drawable Image content within the designated space, (e.g. left, right or center)

  3. Field Width: Here you can adjust the length of your Drawable Image box, (e.g. with the Full option, the image will run the full length of the form, and with the Half option, the image will run to half of the page, leaving room for another form field.)

4. When you’ve finished configuring the Drawable Field settings, click the OK button.

5. The Drawable Field content will appear in the Step Designer. Drag the Text Field to where you want it using the drag-able icon on the top left of the bar.

Once you’re done designing your form, don’t forget to hit the Save Form button at the bottom left of the page.
