How to Link a Form to a Contact

Tired of wasting time, manually inputting patient information into their contact record? With Aloha, we can connect with specific patient management systems to automatically transfer form information directly to the patient's profile once submitted!

*EHR must be integrated with Aloha, some exclusions may apply

Automatic Form Links

For the form to connect to the patient record automatically, the link to the form will need to include a contact verification token. This is an individual identifier that is unique to each contact record. To do so:

  1. Get the link to the direct page the form is on from your site.

  2. Add the link to the template (campaign, bulk message, saved message, or conversation thread).

  3. Add the replacement string "~Contact.VerificationToken~" to the end of the link. (ex. https://mysite.com/new-patient-paperwork/?~Contact.VerificationToken~)

  4. Once sent to the patient, they should be able to click the link and submit the form. It should automatically connect to the patient's contact record.


Manual Form Links

If the patient does not click the link with the contact verification token attached, the form will not automatically connect to the patient record. Rest assured, the submitted form will still be saved under the form submissions within your Aloha account. To manually connect the form to the patient:

  1. Go to FORMS > SHOW FORMS.

  2. Click the number under the "#Submissions" Column.

3. Select the form that the patient completed.

4. Use the "Link Contact" Search bar on the top left to search for the patient's name and select their name.

5. Once their name is selected it should link the form to the patient and it should transfer to your EHR within a couple of minutes.

* If a patient does not at least have a phone number or email attached to their contact, their name will not show on this list.
