✅ Steps to Enable Forms on Your Hosted Site
Follow these steps to make sure your forms show up where your patients can find them:
Open the Aloha Dashboard and go to the Hosted Site Editor
Click the Settings Wheel ⚙️ in the top-right corner of your dashboard.Select "Hosted Site"
This brings you into your site editor.Click on the "Forms" tab at the top of the editor
You’ll land on a page where you can manage form visibility.Click the Settings Wheel ⚙️ next to the “Forms” section
This opens the form selection window.Click “Select Forms”
Choose which forms patients should be able to access from your site. You can select one or more.Click the blue “Select Forms” button at the bottom to save
This confirms your selections and closes the pop-up.
Click “Save Changes” in the top-right corner of the page
This step is crucial—without saving here, your updates won’t go live!

The Forms page on your hosted site now displays your selected forms
Patients can open, complete, and submit those forms online
