Good news - "Always Allowed" hasn't disappeared. It's hidden behind a single iOS setting, and it takes about ten seconds to bring back.
Why this happens
Apple's Screen Time has a master toggle called "App & Website Activity."
When it's turned on, iOS shows "Downtime," "App Limits" and "Always Allowed" as separate rows under "Limit Usage."
When it's turned off, iOS collapses all three into a single row labelled "App & Website Activity" with the subtitle "Reports, Downtime & App Limits." So "Always Allowed" is still there - it's just not visible until the toggle is back on.
How to fix it
Open "Settings" on your iPhone
Tap "Screen Time"
Tap "App & Website Activity"
Turn it on
Go back - "Downtime," "App Limits" and "Always Allowed" will now appear under "Limit Usage"
