​Step 1. Guardians of Minors can reach out to the Safe Space Support team to obtain a copy of the Parental Consent form

Step 2. Guardian is to sign the Parental Consent form and send it to Safe Space Support Team via email at support@safespace.sg

Step 3. Signed forms are to be sent over before the booking of the session.

Step 4. The support team will review the signed form and ensure details are filled in accurately.

Step 5. Once the form has been reviewed, the support team will inform the client to proceed with the booking.

Step 6. Account for Minor to be created and sessions to be booked with Minor’s account.

Step 7. Proceed with the same booking steps with vouchers (If any)

Step 8. The therapist may reach out to the Support Team to obtain the copy of the signed Consent Form.
