Completing a MedsCheck

A Step-by-step guide on how to complete a MedsCheck service and Submit to the PPA Claims portal

REQUIREMENTS:


Completing a MedsCheck

1. FIND PATIENT

a. In ScryptHQ > Search for your patients name in the search bar

b. Select the patient’s profile

c. Open up the “New Service” side blade on right side of the screen

2. SELECT TYPE OF MEDSCHECK
i.e. Regular Medscheck or Diabetes MedsCheck

3. PATIENT DETAILS

a. Confirm the displayed details

b. Select YES for "Reviewed By Pharmacist?"

4. CONSENT & ELIGIBILITY

a. If the patient has provided consent, select Yes

b. How has patient given consent, Select Written or Verbal

c. To upload a saved consent form, select browse and select your saved consent

from your computer

d. Select Upload Files

5. PHARMACY INFORMATION

a. From the dropdown menu, select the Pharmacists name completing the Service

Note: If you don’t see your name, follow the “add pharmacist into ScryptHQ” procedure – this need need to be done in each pharmacy that you service.

6. MEDICATION PROFILE

a. Enter in Allergies and adverse drug reactions (if known)

b. Enter in their Primary Doctor (GP) (If known)

c. Systematically go through each displayed medication and complete as necessary.

d. If the customer is not taking the medication, untick the box corresponding to the ceased medication

e. To add more details to a medication, select the pencil

f. Inside medication details - add any additional information as required

g. Save Medication

7. ACTION PLAN

a. If no action(s) are required – select No Action(s) Take and go to Step 8.

b. If there are issues that need to be added to the action plan – select Add Issue(s) and fill out the details as required

8. SUBMIT

a. PPA submission acknowledgment – Select Submit

9. PATIENTS REPORT (MEDS LIST)

a. You can chose to print Meds List; or

b. SMS/Email the Meds List to the patient by selecting Send Meds List

10. CREATE CRM NOTE

a. Select a follow up date and add any information required for the follow up - We've set up the dates to be a 12-month reminder to be in line of when the customer might be due for their next MedsCheck Service.
