STEPS

  1. Click S2Partner

  2. Click menu open

  3. Click Supplements

  4. Click menu closed

  5. Click Custom Questionnaires

  6. Click Questionnaire Name

  7. Click Question

  8. Click +

  9. Scoring for questionnaires is reliant on scoreable questions
    ​PLEASE NOTE: Only certain field types have scoreable responses

  10. Click Field Type

  11. Select Scoreable Field Type

  12. Enter Nickname
    ​Keep it simple

  13. Options
    ​Create response options for the respondent to choose from. Each response option should have a score or point value

  14. Enter Text

  15. Enter Score

  16. Click +

  17. Repeat
    ​You need at least 2 options to have a score spread

  18. Click Save
    ​When you're ready

  19. Fill in the required fields

  20. Click Save

  21. Click Scoring

  22. Review Minimum & Maximum Values
    ​Maximum Value must be higher than 1 to have a point spread

  23. Select Score Type
    ​This is how the questionnaire will be scored. The following score types are supported:
    ​- Total
    - Percentage
    - Fraction
    - Categories

  24. Review Format & Example

  25. Click Save
