Disability Placards can be issued to individuals of any age with a qualifying disability, as confirmed by a physician on the Disability Placard Application. This applies to both temporary and 5-year placards, which are issued to the person, not a vehicle. Conditions like severe arthritis, respiratory diseases, and mobility impairments are examples of qualifying disabilities.

To apply, complete the application and have it signed by a physician, then submit it to Service Oklahoma in person or online.
