To ensure proper delivery, please make sure that you’ve given us your correct mailing address. Also, be sure that you’ve included any apartment numbers, suite numbers, or box numbers in your return mailing address. If you haven’t received your disability parking placard after 2 months, you’re welcome to call the Disability Parking Permit Desk at (405) 425-2693 or toll free at (833) 306-0128 to check on the status of your application.
