If your district uses HBS as its Medicaid billing vendor, your district may have SLP Toolkit set up to collect documentation for Medicaid billing. In this setup, you’ll enter all required details in Toolkit, and your district plan manager will generate the billing report to share with HBS.
👉 Important: Not all districts using HBS have this integration set up. Some districts still require providers to document directly in HBS’ platform. Please confirm with your district Medicaid coordinator or plan manager to know which process applies to you.
If your district is using SLP Toolkit for HBS billing, follow the documentation requirements below to make sure your sessions are eligible for Medicaid claims.
Documentation Requirements for HBS
Provider Information
Full Name: Use your legal name as it appears in your district employee records.
Employee ID: Enter your license number.
Student Information
Full Name: Use the student’s legal name.
Date of Birth (DOB): Add the student’s date of birth.
DX Code (ICD-10): Select the ICD-10 diagnostic code that applies.
School District Name
School District ID
Therapy Session Notes
Goal: Document progress toward an IEP goal and/or add a note on a goal addressed in the session.
Notes: Include a description of the activity completed during the session.
Data for Billing Export
Service Type (CPT Code): Select the correct procedure code for the service.
Place of Service: Indicate whether the session occurred in school or via telehealth.
Best Practices
✅ Double-check all entries for accuracy and completeness.
✅ Always use full legal names (no nicknames).
✅ Ensure ICD-10 codes match the student’s Medicaid eligibility documentation.
✅ Confirm your district’s School Name and School District ID fields are correct.
What Happens Next
If your district has enabled SLP Toolkit → HBS billing, your district plan manager will run and submit the HBS billing report. You do not need to take additional steps beyond documenting accurately in Toolkit.
