First, navigate to the Officials (whistle) tab on the left side bar. This will bring you to the Officials page. Here you will click on Observations on the side menu.
Clicking into Observations gives you the options to view/edit existing Observation Forms and to create a new Form. Do this by clicking the green box in the top right corner (+Create Form).
In this Observation Form you are able to name it, set the status of this form, choose up to 2 activities that this form can be used for, and then set up your Evaluation categories and ratings.
