To be eligible for reimbursements through your company's HRA, you must have qualified health insurance coverage.
For QSEHRA, you must have coverage that qualifies as Minimum Essential Coverage.
To ensure your company remains compliant with HRA guidelines, we’ll request proof of your qualified health insurance coverage at the following times:
During onboarding – when you first join our platform to begin your QSEHRA or ICHRA program
At each compliance period – either quarterly or annually, depending on your plan
Whenever your coverage changes – including updates, renewals, or plan switches
Providing this documentation helps us verify your eligibility and maintain compliance with HRA requirements.
What to provide:
In order to be compliant, we will need to see a document or combination of documents with all of the following information:
You can also consider submitting an insurance ID card that displays your name, carrier, plan, and enrollment details. If specific information like enrollment dates is absent on the ID card, you may submit additional supporting documents such as a benefits summary or an enrollment confirmation to fulfill required information criteria.
Your name - (The HRA-eligible person - whether you're the primary policyholder or a dependent)
The premium amount - (Only required if you're submitting it for a reimbursement)
A current date - (Document dated within the last 30 days are considered current)
Your plan and insurance provider - (For example: Gold/Silver/Bronze plan name along with the insurance company)
If your primary document is unavailable or incomplete, consider uploading substitutes such as plan invoices, billing statements, or proof of recent premium payment to confirm the coverage details requested.
While it is our preference to have all of the above information on one document, we realize that is not always possible. Multiple documents are acceptable as long at the combination of documents provides all the above information we are asking for. For instance, you could pair a billing statement that shows your premium amount with an official plan document listing the effective coverage date to meet the required criteria.
Where & How to Upload a New Proof of Coverage
You can upload a new proof of coverage through your member portal in the Health Insurance section.
Then go to Edit in the top right-hand corner:
If the Edit button is greyed out or not visible, your document may currently be under compliance review. Wait for the review to complete before re-uploading, or contact Customer Experience if you need to make an urgent correction.

Select your Tax Credit information:
Let us know who the plan is for, how much the premium is, and the dates that the plan is in effect. Note: if you do not want to be reimbursed for the premium amount, put $0.00 into the premium field.
Upload your new proof of coverage. You can drag and drop the document right onto the webpage, or click browse to find it in a folder on your computer!
After uploading your updated Proof of Coverage, click continue. This will send the information to our Compliance team for review! They will send you an e-mail letting you know if your document was approved or if we need additional information before approval.
If your proof of coverage submission is declined, revise and upload new supplemental documents ensuring they include any previously missing information, such as an ID card displaying your plan name alongside other existing materials.
Just a reminder: files must be 10MB or smaller. While PNG is the preferred format, PDF and JPEG files are also acceptable. HEIC files (the default photo format on iPhone and iPad) are not supported — convert to JPEG, PNG, or PDF before uploading.
