Your Vision plan reimburses 90% of the Usual, Customary, and Reasonable (UCR) cost for the service—not necessarily the full amount your provider charged.

  • Usual and customary costs are the typical prices for a service in your area.

  • If your provider charges more than the typical cost, your reimbursement will be based on the UCR amount, not the full billed amount.

  • This means you may be responsible for the difference between what your provider charges and what your plan reimburses.

This is standard for most vision plans, so even if the amount seems lower than expected, it reflects the plan’s reimbursement rules rather than an error.
