Vitable’s vision benefits are designed to help you maintain good eye health and access affordable vision care. This plan includes annual exams and allowances for frames, lenses, and contact lenses. Enhancements such as polycarbonate lenses for children, anti-reflective coatings, and scratch protection are also included.
What’s Included:
Annual eye exam — Covered in full after a $15 copay
Eyeglass lenses — Single vision, bifocal, trifocal, lenticular, and progressive lenses all covered 100% after a $15 copay
Lens enhancements — Includes scratch-resistant, ultraviolet, and anti-reflective coatings
Eyeglass frames — Covered up to $800 every two calendar years
Contact lenses — Covered up to $800 per year in lieu of glasses (includes a 12-month supply of disposables or a pair of therapeutic lenses if medically necessary)
Key Notes:
Coverage includes either one pair of eyeglasses or one set of contact lenses per year, not both
Lens benefits include polycarbonate lenses for children under 18
Some restrictions apply for therapeutic contact lenses, based on medical need
What network do I have access to?
