Below we've provided a library of example questions. There are 2 question types available:

  • Free-text - employees leave comments

  • Sliding scale - provide scores of 0-10. The 3 labels Never/Sometimes/Always are automatically applied to the scale, but you can change these.

Please note: When replacing our automatic scale labels Never/Sometimes/Always, do not leave any of the boxes blank as it will use the existing label. If you don't need a middle label, simply put a '-' in the box. (see image below).

We have organised the below questions by Topic and Question Type.

For Sliding Scale questions, in brackets we've provided the 3 scale labels you can use to replace our existing labels e.g. (Strongly Disagree/-/Strongly Agree)
