How to send a Performance Review questionnaire

Within the Perform module of WeThrive, you have the ability to send an individual, or a group of people, a set of questions to review their performance ahead of your next review, appraisal, coaching session etc.

This help file walks you through how to set up and send a questionnaire to an individual and to a team if you want to standardise your review process.

At the bottom of page is some further information on anonymity as well as a full video walkthrough on sending a Performance Review to an individual or team.

Send a Performance Review questionnaire to one individual

  1. Go to Reviews > Individual Reviews tab > click the Send button against the individual.

  2. Add a Title and optional Description to appear at the top of the questionnaire.

  3. The questions included in your questionnaire display in the box on the right labelled Selected custom questions. Edit the question set in the following ways:

    1. WeThrive's recommended question set will appear in orange. Click Edit next to each one to customise/change the question.

    2. Remove questions by clicking the '-' [minus sign] next to each one.

    3. Add your own questions using the Add new question button.

    4. Re-order your questions using the arrows.

  4. Before scheduling, preview your question set via the button in the right hand corner. You can also preview the email wording that will be sent to employee(s) with their link when it goes live.

Once you're happy with your question set, click Send at the bottom of the page. This will immediately send an email to the individual with a link to complete their questions.

Send a Performance Review questionnaire to a group of people

  1. Go to Reviews > Team Reviews tab > click Schedule performance review to send the same question set to a group.

  2. Enter a name for your review, start date, start time and duration.

  3. Choose which automated email notifications you'd like to include by ticking the boxes next to each one.

  4. Click the Next (custom questions) button at the bottom of the screen.

  5. Add a Title and optional Description to appear at the top of the questionnaire.

  6. The questions included in your questionnaire display in the box on the right labelled Selected custom questions. Edit the question set in the following ways:

    1. WeThrive's recommended question set will appear in orange. Click Edit next to each one to customise/change the question.

  7. Before scheduling, preview your question set via the button in the right hand corner. You can also preview the email wording that will be sent to employee(s) with their link when it goes live.

  8. When you're happy with your questionnaire set up, click Next (select people) at the bottom of the page.

  9. Choose who you want to send the questionnaire to either by clicking Select All at the top of the page, or ticking the box next to certain individuals. Use the filters to help you find individuals more quickly.

  10. Click Review settings at the bottom of the page. Here, you can double check the details and go back to edit your questions via the View/Edit button under the 'Custom questions' heading.

  11. Customise your email template wording by clicking the link next to each email notification you are including.

  12. Once happy, click Schedule review. This will schedule your questionnaire to be sent to selected individuals at your chosen date/time.

If you chose to include a warmup email, you can choose when this will be sent by customising the warm up email template. The warm up will go out straight away if the launch date is less than 24 hours away.

Are results anonymous?

No, results to performance review questionnaires are always non-anonymous due to the nature of the topic. A performance review is specific to one individual, evaluating their own experience, performance and goals, so cannot be anonymous as the results are designed to inform manager-employee discussions including 1:1s, appraisals, quarterly reviews, annual reviews etc.
