We need to see extra documents for Flu and Covid shifts, you can see the full list below.
Make sure to keep yourself up to date with any trainings, as these are subject to updates. Covid Moderna training from 2021 is now out of date due to changes in the training module, and we will need to see a 2022 copy.
Flu
Vaccination Declaration of Competence (email back)
In-person immunisation training certificate (incl. BLS and anaphylaxis training - if not included, separate evidence of this training is required)
Covid
COVID Vaccination Declaration of Competence - 23/9/2022 and onward (if completing this, you don't need to also provide the above DoC)
