Notes can be added in the PDF editor either at a document level or bundle level. More guidance on opening documents or bundles in the PDF editor can be found here.
With your document open in the PDF editor, first select the "Note" tool in the ribbon.
With the note tool selected, click anywhere on the PDF to add a note to that location. You can then type in the note contents.
Before adding a note to the PDF, you can change the note colour, and icon using the sub-ribbon.
