1. Select "Served".
2. Select which type of service it was (personal or substitute).
3. "Was Service Accepted?" - Select "Tried to Refuse Service".
4. "Tried to Refuse Service By?" - Select what applies.
5. "Tried to Refuse Service Because?" - Select what applies.
6. "Did You Leave the Documents?" - Select "Documents left, seen by subject".
