Aethair CMS-2567 Reports

Document whether healthcare environmental conditions stayed within the limits CMS surveyors check

Aethair CMS-2567 Reports show whether environmental conditions in your healthcare facility stayed within the limits that CMS surveyors check. These include humidity in operating rooms, temperature in clinical areas, pressure in isolation rooms, and indoor air quality. Each condition gets a clear Compliant, Non-Compliant, or Caution status. Violations and data gaps are listed by device, so you can document conditions and act before a survey.

CMS-2567 is the Statement of Deficiencies and Plan of Correction form that CMS issues when a facility fails to meet federal Conditions of Participation.

For the settings every report shares (name, schedule, Noesis summary), see Creating & Scheduling Reports.

The Aethair Environmental Compliance Codebook (AECC)

The report checks your data against the Aethair Environmental Compliance Codebook (AECC). Each AECC code represents a specific compliance condition aligned with CMS requirements and healthcare accreditation standards. The codes are grouped into four categories:

Code range

Category

What it checks

AECC-100

Humidity Compliance

Relative humidity in operating rooms, sterile storage, and critical care areas

AECC-200

Temperature Compliance

Temperature in clinical spaces

AECC-300

Pressure & Ventilation

Positive pressure in operating rooms and negative pressure in isolation rooms

AECC-400

Air Quality

PM2.5, PM10, VOCs, and CO₂

Set up an Aethair CMS-2567 Report

  1. In Reports, click + New Report.

  2. Fill in the Info card.

  3. In Settings, set Report type to Aethair CMS-2567. Section 2 will change to AECC Compliance Categories, with a dropdown to select the code range.

  4. To add more categories, pick one from the Add AECC category dropdown. It only lists categories you haven't added yet.

  5. For each category:

    • In Devices, choose the devices that monitor those spaces. For example, choose only the devices in operating rooms for Pressure & Ventilation. To report on rooms with different requirements, such as ORs and isolation rooms, create a separate report for each.

    • Under Subcategories, check the codes that apply to those spaces. Use Select All or Select None to change them all at once.

    • Click ⓘ next to a subcategory to see its limits.

  6. To take a category out of the report, click Remove. A report needs at least one category, so Remove is unavailable when only one is left.

  7. Set the Content and Time Range & Schedule cards on the right side, then click Create. CMS-2567 reports support the same options as other reports, including Working hours, the Noesis summary, and scheduling.

AECC codes and limits

The limits are fixed by the AECC and can't be edited. A violation is recorded when a reading stays outside the limit for 5 continuous minutes.

Code

Subcategory

Compliant range

Applies to

AECC-101

RH Out of Range (Critical/OR)

Humidity 20–60 % (per ASHRAE 170)

Critical care areas, ORs, ICUs, sterile corridors, procedure rooms

AECC-102

Sterile Storage Humidity Failure

Humidity ≤ 60 %

Sterile storage and sterile supply rooms

AECC-201

Temperature
Out of Range

68–75 °F (per ASHRAE 170)

Clinical areas

AECC-301

Positive Pressure Failure (OR)

Differential pressure > 0 Pa

Operating rooms

AECC-302

Negative Pressure Failure (Isolation Room)

Differential pressure < 0 Pa

Isolation rooms, hazardous drug rooms

AECC-401

Elevated
PM2.5 / PM10

PM2.5 ≤ 12 µg/m³ and PM10 ≤ 25 µg/m³

Clinical spaces

AECC-402

VOC Spike / Chemical Exposure

VOC ≤ 0.25 ppm
(250 ppb, isobutylene equivalent)

Clinical spaces

AECC-403

CO₂ Ventilation Indicator Failure

CO₂ ≤ 1000 ppm

Clinical spaces

AECC-402 accepts VOC readings in ppb, ppm, or ppm isobutylene, so it works with different VOC sensors.

Compliance statuses

Status

Meaning

Compliant

Every reading was within the limits, and no data is missing.

Non-Compliant

At least one reading was outside the limits. The report counts the occurrences and their total time.

Caution

No readings were outside the limits, but data is missing for part of the period (a data gap).

A category's status reflects its subcategories. If any subcategory is Non-Compliant, the whole category is Non-Compliant.

What's in an Aethair CMS-2567 Report

Header

The report name with a Compliance · CMS-2567 label, plus the account name, reporting period, working hours (if set), and time zone.

AECC Category Groups

A table with the status of each category in the report. It lists the code range, category, description, and a status icon. Below it is a 'Remarks' box for additional comments and a short definition of the AECC.

Category pages

Each category has its own page with its overall status at the top. Each subcategory shows:

  • Description – the requirement being checked

  • Trigger – what counts as a violation (5 minutes continuously out of range)

  • Violations – a card for each device with a problem. It shows the device name, serial number, and parameter, plus either:

    • Red card: the number of out-of-range events and their total time,
      e.g. 288 events • 1 day

    • Yellow card: how long the data gap lasted, e.g. Data gap: 1 day

If a subcategory had no problems, Violations shows None.

Some examples below.

Summary by Noesis

If Include Summary by Noesis is on, the report ends with an AI-written summary and key findings.

Tips

  • Resolve Caution statuses. Data gaps are hard to defend in a survey. Check device connections and power with Troubleshooting Devices.

  • Schedule it. A weekly scheduled report builds a running record you can hand to surveyors.

  • Get alerts in real time. Set up alerts with the same limits, so your team can fix problems as they happen instead of finding them in the report.

Note: AECC limits help you document environmental conditions. They don't replace your facility's own policies or a surveyor's findings.
