In the bottom left hand corner, click on Settings -> Organisation Settings -> Audit and reporting.

Here you can search audit logs of activities carried out by members of your organisation and download them to your computer as in .csv format. This can include changes made to requests, as well as organisational setting changes.

You must set a start and end date and can only fetch up to 10,000 entries at once. You can use the optional filters to filter your results by users at your practice or by NHS number to see activites relating to a specific patient.
