How do I add running headers/footers in the ePub?

Unlike printed books, eBooks do not typically include pre-programmed running headers or footers within the file. While certain eReaders may be programmed to automatically generate running headers displaying the title and/or author name, these headers are not intrinsic to the eBook file itself.
