How to use the Resident Assessment

This video demonstrates how to use Resident Assessment to determine a resident’s needs and level of care with ADLs to create Service Plans and Care routines with community-specific details.


Step 1.

In the resident’s Documents tab, click on the Resident Assessment under New Document.

Step 2.

Under the Tasks tab, open the Resident Assessment.

Step 3.

Complete all of the sections in the assessment for the ADLs. These will calculate the Care Score, Level, and Fall Risk Score and flow into the Service Plan.

Note: Checking the morning and bedtime boxes for Dressing, Grooming, and Oral Care will create Care routines for Morning and Bedtime.

Step 4.

Review the completed Resident Assessment and scores, then “Finalize” or “Make Changes” to the assessment.

Step 5.

When the Resident Assessment is completed, you will have the option of creating a new Service Plan task at that time or selecting “Not Now”.

Step 6.

Choosing “Yes, Create Task”, confirms you have completed the Resident Assessment and created a new Service Plan task on the resident’s Tasks tab.
