How to Add COVID Vaccination Dates

Adding COVID-19 Vaccination information will reflect on your Face Sheets

You can add COVID-19 records either from a resident's Details tab or their Documents tab:

Use the Details tab

  1. Click on the "Details" tab

  2. Hit the “Add” button in the top right corner of the "COVID-19 VACCINATIONS" card to add your records:

  3. Fill out the required fields around date, manufacturer, and dose, and add any notes like "Booster shot #2" if needed. Then, hit save in the bottom right corner:

  4. Add additional doses by repeating steps 2 & 3 as needed.

  5. We recommend that you use the “Upload Document” button on the black box to upload an image of the COVID vaccination card. This can be done for any dose & only needs to be done once:

Use the Documents tab:

  1. Click on the "Documents" tab

  2. Click the button on the bottom of the right-hand "New Document" menu:

  3. Select "COVID-19 Vaccination" as the document type:

  4. Repeat steps 3 & 4 in the section above to add additional doses and upload images of the vaccination card.
