Add an AHTR form

You can easily create and submit an Allied Health Treatment Request (AHTR) form through your Avaana Business Dashboard. Follow the steps below:

Navigate to Client List

  • Log in to your Avaana Business Dashboard.

  • From the left-hand menu, select Client List.


Search for Your Client

  • Use the search bar to quickly find your client, or scroll through the list to locate them.

  • Once you’ve found the client, click on the three dots menu (⋮) next to their details.


Select Submit AHTR

  • From the dropdown menu, choose Submit AHTR.


Complete the AHTR Form

A new form titled Allied Health Treatment Request will open.

  • Fill in all required fields (marked with a red asterisk *).

  • The form is divided into 6 sections:

    1. Injured Person Details

    2. Your Clinical Assessment

    3. Barriers to Recovery and Strategies to Address

    4. Treatment Plan

    5. Services Requested

    6. Service Provider Details

Make sure all details and your signature are completed.


Review & Submit

  • Double-check all details entered for accuracy.

  • Once ready, click Submit Form.

And that's it!

Your AHTR has been submitted.

Post submission, the form will be automatically shared with the relevant insurer, and they will reach out to you with the response, we'll be adding additional appointments once they're approved.

The current limit of appointments are 8 for Physiotherapy and 6 for Psychology, for additional appointments an AHTR submission is mandatory.
