Add a Cost Recovery form

How to add a cost recovery form.


Find Cost Recovery form

  • Go to Client List

  • Click the 3 vertical dots on the relevant client

  • Click "Add Cost Recovery"

  • This will open up the Cost Recovery form page


Incidental Expenses

Select the following service depending on the modality of service:

  • Physiotherapy, select the service "Incidental Expenses (PTA020)"

  • Psychology, select the service "Incidental Expenses (PSY020)"

In the "Incidental Expenses" row, enter the product name, quantity and unit price

Please note: Incidental expenses above $110 per claim requires insurer pre-approval.


Once you have input the incidental expenses, review the information is correct, then to submit the form, check the boxes and click "Submit"


Report Writing

Select the following service depending on the modality of service:

  • Physiotherapy, select the service "Report Writing (PTA016)"

  • Psychology, select the service "Report Writing (PSY003)"

In the "Time Spent" field, enter the duration of time spent writing.

Please note, maximum of 1 hour of report writing is chargeable.

Once you have input the report writing duration, review the information is correct, then to submit the form, check the boxes and click "Submit"
