Personalize your forms, performance logs and post-workout questionnaires using the BridgeAthletic Form Builder

Are you looking to build your own forms or to customize your daily readiness questionnaire? This article walks you through the BridgeAthletic Form Builder, which allows you to (1) build your own forms and questions, (2) customize an existing form

Note: this is only available to Sports, Tactical and Enterprise Private accounts.

1) Build your own forms and questions

  • From your homepage > Forms > New Form

  • Choose the desired form type

    • Daily Performance Log: filled out daily by your team

    • Post Workout: filled out after each workout or session

    • General Form: one-time survey sent to your athletes

  • Name your form and add a description (optional)

  • Add your questions by

    • (a) Creating a New Question

    • (b) Adding an Existing Question.

Bonus Tip #1: "Add Existing Questions" is where you will find all the questions you have created previously--you can use them at any time and in any form!

A breakdown of our available question types:

  • Multiple Choice

  • Checkboxes

  • Number

  • Open Text Box

  • Range

  • Range with Tooltips

  • RPE

  • Duration

  • Bodyweight -- Automatically updates your athlete's body weight on their profile page

  • Height -- Automatically updates your athlete's height on their profile page

2) Customize an existing form

Either build a new form from scratch (see above) or clone to edit an existing form.

  • From your homepage > Forms

  • Select a form to edit > Click on the 3 dots > Clone

  • Rename your form and click on Edit to start customizing your form!

Bonus Tip #3: Editing existing questions and answers will override data already gathered. We recommend you clone any assigned forms before making edits.
