Building and Customizing Forms with BridgeAthletic

Are you looking to create your own forms or customize your daily readiness questionnaire? This guide walks you through how to do this in the BridgeAthletic Form Builder.

The number of custom forms you can create depends on your subscription package. If you need to increase this limit, please contact support@bridgeathletic.com.

1) Building Your Own Forms and Questions

Steps to Create a Form

  1. Navigate to Homepage > Forms > New Form

  2. Choose a form type:

    • Daily Performance Log: Filled out daily by your team.

    • Post Workout: Completed after each workout or session.

    • General Form: A one-time survey sent to your athletes.

  3. Name your form and optionally add a description.

  4. Add questions by:

    • Creating a New Question - see the list below of question types available

    • Adding an Existing Question - previously created questions can be reused at any time

  5. Editing After Completion: If you need to make adjustments after creating a form, you will need to toggle on Edit Mode.

Question Types Available

  • Multiple Choice

  • Multiple Choice with Values

  • Checkboxes (with min/max values)

  • List of Sports

  • Number

  • Dropdown List

  • Range (with optional tooltips)

  • Height

  • Bodyweight

  • Net Promoter Score (NPS)

  • Media

  • Contact Number

  • Open Text Box

  • Date

Form Layout and Organization

Proper form layout ensures clarity and ease of completion. Follow these best practices:

  1. Set Required Questions

    • Mark essential questions as required to ensure completion before submission. This is available when editing the question.

  2. Use Sections to Improve Organization

    • Select to add a section at any point to break a form up.

    • Name your section and then add your questions to this new section.

    • Breaking up forms into sections is a great tip for longer forms.

  3. Utilize Drag-and-Drop Functionality

    • Easily reorder questions by dragging and dropping them into the correct sequence within each section.
