How To Create and Customize a Questionnaire

Collect important trip details from clients before their trip. Build custom questionnaires, organize them with layouts, and attach them to experiences so they appear automatically in the client portal.

Questionnaires help you gather key information before a trip—like dietary restrictions, health concerns, guide preferences, waivers, or special requests.

They are attached to experiences, so once a trip is confirmed, the questionnaire automatically appears in the client’s portal and itinerary.

You can fully customize your questionnaire using fields and layout tools to keep everything clear and organized.


How to Create a Questionnaire

Step 1: Open Questionnaires

  1. Go to Product Settings

  2. Click the Questionnaires tab

  3. Click + Add a Questionnaire


Step 2: Add Basic Details

  1. Enter a Title

  2. Add a Description (optional but recommended)

  3. Start building your form


Adding Fields (Questions)

Click + Add Field to insert questions into your form.

Available Field Types:

  1. Single Select – Choose one option

  2. Multi-Select – Choose multiple options

  3. Dropdown – Select from a dropdown list

  4. Date – Select a date from a calendar

  5. Monetary – Enter a dollar amount

  6. Percentage – Enter a percentage value

  7. Number – Enter a numeric value

  8. Checkbox – checkable option, useful for agreements or acknowledgments

  9. Short Text – Brief response

  10. Text Area – Longer written response

Each field includes:

  • Question Text – The main question

  • Help Text – Optional extra explanation

You can add as many fields as needed.


Organizing Your Questionnaire with Layout

To keep your form clean and easy to follow, click + Add Layout and choose from:

  • Header – Add a section title to group related questions

  • Block Text – Add instructions or important information (no response required)

  • Divider – Visually separate sections

  • Group – Bundle related questions together

  • Columns – Display questions side-by-side

Using layout tools helps you:

  • Break long forms into clear sections

  • Add instructions before certain questions

  • Keep everything organized and professional

TIP: Use headers and dividers for longer questionnaires to make them easier for clients to complete.


Step 3: Save Your Questionnaire

When finished, click Save in the top-right corner.


Using a Questionnaire as a Pre-Confirmation Task

Questionnaires can also be assigned as a pre-confirmation task on an invoice, requiring clients to complete the questionnaire before they can submit payment. This is useful for collecting essential information upfront as part of the booking process.

  2. Manage Questionnaires – Edit, duplicate, archive, or restore as needed

  3. Attach to an Experience – Add as required or optional so clients see them post-booking

Use this flow to gather the right info automatically—before every trip.


Important: After creating a questionnaire, be sure to attach it to an experience—otherwise, it won't appear for clients. If you allow minors as trip participants, also review the questionnaire's Age group setting on the Questionnaires tab so it's required for the right participants.
