Understand moderation results

What Approved, Rejected, and Needs review mean, how decisions are made, and how to trace them

Every moderated asset gets a clear outcome, and every outcome can be traced back to the rule that caused it. This article explains what each status means, how to read rule-level results, and what to do when you disagree.

The three asset statuses

  • Approved - the asset passed all the rules in the run. It's ready to use.

  • Rejected - the asset clearly failed at least one rule. The failed rule and its rejection reason tell you why.

  • 🚩 Needs review - the result wasn't clear-cut. No rule failed outright, but at least one wasn't confident enough to decide automatically. A person should look and decide.

An asset must pass every rule to be approved. One clear failure is enough to reject it - even if ten other rules passed.

How a decision is made

Each rule scores the asset and compares the score against the rule's Review threshold (set when the rule was created):

  • Scores clearly on the "pass" side → approved automatically for that rule.

  • Scores clearly on the "fail" side → rejected automatically, with the rule's rejection reason attached.

  • Scores in between → sent to review.

That's why the same image can be approved by a lenient rule and flagged by a strict one - strictness is a per-rule setting you control.

Reading rule results on an asset

Select any asset in a run and open the Details tab in the sidebar. Under Rule Results, for each rule you'll see:

  • The outcome - Pass, Fail, or Review for this rule.

  • A confidence score - how sure the system is about its finding (shown as a percentage).

  • An explanation - what was actually found, in plain language. For videos, explanations can include timestamps you can click to jump to the exact moment.

  • Rejection reasons - for failed rules, the reason configured on the rule (for example, "Watermark detected").

Why did this asset get this result?

To trace a result:

  1. Open the run and select the asset.

  2. In the Details tab, scan the rule results for the one that failed or was uncertain.

  3. Read its explanation and confidence score.

  4. If it's a video, click any timestamp in the explanation to see the moment in question.

  5. Check the History tab to see the full decision trail, including any manual decisions made since.

When you disagree with a result

You're always in control:

  • Approve or Reject the asset manually - from the sidebar or in Preview mode. Your decision overrides the automatic result and is recorded with your name in the asset's History.

  • Clear Manual Decision - reverts the asset to its automatic result if you change your mind.

  • If a rule keeps producing results you disagree with, adjust its Review threshold (make it stricter or more lenient) or refine its configuration, then test with a new one-time run. Existing results don't change when you edit a rule — only future runs use the new settings.
