Introduction

This article will show you how to create a Scored Survey and Feedback in Cogniss Creator. You can set up scores for individual options of a survey question and provide personalized feedback based on the user's score.

Step 1: Create a Scored_Survey type Topic

To give a score to each survey question (or option), the Topic needs to be defined as Scored_survey

  • If you are creating a new Topic, select Scored_survey as the Topic type

  • If you already have an existing Topic, go to the Topic editing page (Subject > Topic tab) and change Survey to Scored_survey

Step 2: Set your score to the survey question options

Once your Topic type is defined as Scored_survey, do the following:

  1. Create a Content Block with your choice of question type (e.g. multi choice, multi-select)

  2. Add Options to the Content Block. These will be the options/answers you want to provide to your users.

  3. Assign a score to each option/answer with a number value

  4. Repeat steps above for all the options you want to give a score to

Step 3: Set up a Feedback

If you want to send different Feedback messages based on the user's score, you will need to set up different Feedback pages. Each Feedback page will have different score criteria and messages:

  1. Go to Content > Feedback tab.

  2. Create new Feedback with a Title (Feedback description is optional).

  3. Create a new page with a Title, Text and Button label if needed (These will be displayed to the users).

  4. Next, add Criteria to the Feedback page.

To set up Criteria, configure the below options (see the images below):

  • Type: Count

  • Model: ActivityReponse

  • Activity: Select the Activity you set up the score for

  • Score type: You can select from

    • Equal to

    • Greater than

    • Greater than or equal to

    • Less than

    • Less than or equal to

    • Not equal to

  • And put the number value

  • Tick and select Complete status

  • Set a count to 1

Note: Complete means the user has completed that activity and Count refers to the number of times they have completed that Activity (eg. an entire survey).

5. Add another Feedback page if you want to create a different score-based message

6. Once you've finished the page and criteria, save the Feedback.

Step 4: Connect the Feedback to the Activity

Now that you have set up your Scored Survey and Feedback,

  • Go back to the Activity and connect the Feedback you created.

  • After the changes have been saved, the Feedback messages will now appear after a user has completed that Activity, depending on their score.
