How to request patient records

Request Records is the process used to request patient records electronically from any other provider or organization for purposes such as care coordination, workman’s compensation, legal reasons, etc. Request Records uses a comprehensive, but simple, request form to ensure all appropriate request information and consents are included.

While the record request form can be used with any provider using fax or Direct messaging, we have also integrated the system with companies that are contracted to support record request fulfillment for one or more facilities. This allows for advanced functionality, including the ability to present a filtered list of participating facilities to those in your surrounding geography. For integrated facilities, stat requests can also be prioritized to ensure timeliness of delivery for emergent situations.

Request Records

Navigate to Request Records by selecting the Request Records option from the menu on the left. The Sent table includes all requests that have been submitted, including the status and request details. You can view the content of a previously sent request from Message Events.

To open the Request Records form and initiate a new request, complete the following steps:

  1. Select the Request Records button.

  2. The Request Records form will open.

  3. From: Select the address or fax number from which you would like to send this request on behalf of.

  4. Requester: The Requester will default to the user logged in to the system, however, if the request is being performed for another person in the organization, enter their first and last names here.

  5. To: Select a Facility/Hospital if you are geographically close to a desired integrated ROI facility, or manually add an address or fax number in the "Add an address or fax number" field.

  6. Patient Details includes your patient information assigned to the request and the attached consent, including your patient ID or other number used internally at your organization. Complete the following fields:                                                       a. Patient ID/No. (Only one patient can be assigned per request.)                           b. First Name                                                                                                                  c. Middle Name (optional)                                                                                            d. Last Name                                                                                                                  e. Date of Birth                                                                                                                f. Gender

  7. Request Details includes the information regarding why the request is being made, the portion of the patient records desired and the urgency of the request. Complete the following sections:                                                                                 a. Purpose of Disclosure                                                                                              b. Information                                                                                                                 c. Dates Requested (When selecting Specific Date(s), more than one date may be entered, separated by a semi-colon).                                                                          d. Urgency of Request

  8. Additional Requests is used to request access to highly protected portions of the patient record, including behavioral health records, alcohol abuse, HIV, etc. An appropriate authorization must be attached in order to submit the request.

  9. Select the Send button to submit your request, and you will be returned to the Sent table.
