How do I make a referral to Elementary Teletherapy?

HOW TO MAKE A REFERRAL

Check out this quick video walkthrough or follow the steps below!

Use your district email address to log in.

Either enter your password, or if you have a Google-based email address, you can select 'Continue with Google' under the password.

If you have any trouble accessing your account, please reach out to carecoordinator@daybreakhealth.com.

Step 2: Select "Refer a Student" from the homepage:

Step 3: Locate the student's account or enter the student's information into the Daybreak Dashboard.

If your district shared a roster file with Daybreak you will be able to search for the student by name.

However, if you don't see student information in your dashboard, that's ok! Select "Create and refer a student record not (yet) in Daybreak."

You'll be asked to provide the student's contact information, all available in your district's student information system.

Step 4: Fill out the Referral Form

Select the "Elementary Teletherapy" button at the top of the referral form

The Referral Form should take no more than 5 minutes to complete.

Simply enter the student and caregiver's contact information, the primary reason for the referral, and any special considerations that you'd like the Daybreak clinical team to be aware of.

If a student does not have their own email or phone number, please enter the caregiver's information in these boxes.

Examples of Special Considerations:

  • The student or family's language preferences

  • Preferred dates or times for sessions to occur
    ​

Step 5: Submit the Referral

Please select 'Schedule with Parent' to proceed with submitting the referral.

FAQs

What age range does the program serve?

This program is designed for students 5-12.

How long is the program?

This program is up to 12 weeks, with weekly 50-minute sessions over zoom.

Who is this program appropriate for?

  • The parenting skills and child development understanding gained through this program can benefit all parents

  • Consider families who may currently be impacted by additional stressors (e.g. financial difficulties, divorce or adapting to a blended family, death of a loved one) and could benefit from learning positive ways to deal with the impact on both parent and child

  • The program can also help parents support their children through mild to moderate challenges in areas like anxiety and worries, depression and sadness, sleep and bedtime routines, attention and focus, school avoidance, tantrums, and relationship issues

Who is this program not appropriate for?

The program is not appropriate for acute situations including severe abuse, neglect or open CPS cases, active addiction / alcoholism, domestic violence, or mental health conditions that require hospitalization (i.e. suicidal attempts) or are better suited for in-person care.

What will families work on in the program?

Families will work on a custom combination of these 6 learning objectives, based on their individual needs:

Objective 5: Setting Boundaries

Objective 6: Family Systems - Roles & Values

In a two-parent household, do both parents need to attend the sessions?

Both parents are welcome to attend but only one is required.

Do you need consent from both parents?

In the case of shared custody, consent is required from both parents.
