Step 1: Log in to the Daybreak Dashboard at app.daybreakhealth.com.
Use your district email address to log in.
Either enter your password, or if you have a Google-based email address, you can select 'Continue with Google' under the password.

Step 2: Select "Refer a Student" from the homepage:
Step 3: Locate the student's account or enter the student's information into the Daybreak Dashboard.
If your district shared a roster file with Daybreak you will be able to search for the student by name.
However, if you don't see student information in your dashboard, that's ok! Select "Create and refer a student record not (yet) in Daybreak."
You'll be asked to provide the student's contact information, all available in your district's student information system.
Step 4: Fill out the Referral Form
The Referral Form should take no more than 5 minutes to complete. Simply enter the student and caregiver's contact information, the primary reason for the referral, and any special considerations that you'd like the Daybreak clinical team to be aware of.
If a student does not have their own email or phone number, please enter the caregiver's information in these boxes.
Important note on the question 'Parent/Caregiver can be contacted by Daybreak Health?'
If a student is self-consenting please select "No" so that Daybreak will not send information to the parent/caregiver.
All students aged 18+ must self-consent and sign the required forms themselves.

Step 5: Submit the Referral
Please select 'Schedule with Parent' to proceed with submitting the referral.
