Having your child's insurance information on file enables high quality and continuous clinical care. Being able to bill payers allows Daybreak Health to continue our mission and support more families in your community.

If Daybreak bills a student's insurance, will the parent incur any costs such as co-pays?

No. Daybreak's 12-week teletherapy programs are completely free to families.

What is the parent/guardian experience of providing insurance information like?

When a student is referred for care, the parent will receive a link to the Parent Welcome Packet specific to the referral. This packet includes consents required for care, the insurance form if relevant, and option to schedule an intake appointment.

The form will default to the image upload option. Having images of the insurance card is Daybreak Health's preferred method for receiving insurance information as it allows us to more easily manage, validate, and contact payers.

  • Click on the 'Front of Insurance Card' and 'Back of Insurance Card' upload buttons to upload images on file on your device if uploading from a desktop computer or take a picture if filling out from a tablet or smartphone.

  • Fill out any other fields as prompted.

If the parent cannot upload or take pictures of their child's insurance cards, they can upload the information manually as the fields are prompted. Click 'Save' in order to submit insurance information.

What if the student doesn’t have insurance?

If the student does not have insurance, the guardian may click 'My child does not have insurance' and then select they attest their child does not have insurance at this time. The Daybreak Health team may reach out to confirm this information for our records.
