Does Daybreak Health require parents/guardians to sign a Release of Information for services?

Yes.

Why does require parents/guardians to sign a Release of Information for services?

The Release of Information that families sign allows Daybreak Health to be partners with school staff in regard to the logistics of therapy (like where a student is in the referral process, sessions that have been scheduled, or if a youth is struggling to make it to session) as well as in supporting school staff with a youth’s progress at school (like learning about coping strategies the youth is using, and how the school might be able to help). It does NOT mean that the therapist will be sharing details of what is discuss in therapy, as therapy remains a confidential space. The Release of Information also allows for the district, as the sponsor/payer of the sessions, to receive overall data (again about session attendance or people making it through care successfully) but they will not be given the personal information that students discuss in sessions.

What exactly is included in the Release of Information?

Daybreak has created a more limited ROI for youth and families. The areas that the ROI includes are:

  1. Status of Referral

  2. Treatment Dates and Sessions Attended

  3. Treatment Goals, coping strategies that youth may be utilizing at school that staff can support, and broad progress update toward those goals

What if a parent does not want to sign the Release of Information?

If a parent does not want to sign the Release of Information form Daybreak Health will be unable to provide services to the youth.

How will parents provide Release of Information to Daybreak?

Release of Information is included in the Parent Welcome Packet which is emailed and texted to the parent/guardian after a referral is submitted.
