What do the Referral and Onboarding Statuses Mean?

There are different statuses referenced across the dashboard that reflect the different stages of Daybreak Health journey someone might be currently in. Our current statuses are calculations of different inputs that are not necessarily linear, and you can find below the explanation of the statuses and roughly the expected sequential order which describes the stage a youth is in. Please keep in mind that referral and welcome packet status data updates on a 10 minute sync schedule.

Referral Status

‘Referral Status’ refers to the status field displayed on the list of referred students under the ‘Referral status’ column. These statuses are automatically generated based on different inputs being calculated which means they are not necessarily linear, but we have roughly grouped in the sequence you might expect.

Waiting on Parent Welcome Packet

Referral has been successfully submitted and we are waiting for onboarding forms to be completed.

Insurance Info Needed

Insurance information is missing preventing further progress

Waiting for Intake

A live intake has been scheduled already but we are waiting for it to occur. This should be rarely used or needed given our default to the intake questionnaire.

Intake Missed

A live intake was scheduled but was missed. This should be rarely used or needed moving forward as intake questionnaires are the default experience.

Daybreak Working

Daybreak is working through additional manual needs, such as getting additional paperwork or verifying insurance

Pending

This is an in-between status as the Daybreak team manually works the referral across the system and can reflect waiting for a review by an intake specialist, confirmation on legal documentation, etc.

Match in Progress

All information needed from the patient has been collected and verification of information has determined that the youth is eligible for Daybreak care! The team is working on finding the most appropriate, in network therapist for the youth.

Ongoing Care

The youth is scheduled for ongoing care

Closed

Discharged patients will be closed.

Closed: Client not Responsive

Referral is closed due to client not being responsive to onboarding outreach attempts

Counseling Declined

Referral is closed due to client declining daybreak services

Counseling Not Recommended

Referral is closed due to client not being appropriate for Daybreak services

Discontinued Referral Statuses

You may see these referral statuses for referrals made before 1/7/25

Discharged

Referral is closed due to client declining daybreak services, client not being appropriate for Daybreak services, client not being responsive to onboarding outreach attempts, or because the client has completed sessions with Daybreak and is now discharged from the program

Scheduling

Parent needs to complete some or all of the Onboarding Forms

Onboarding Forms Status

‘Welcome Packet Status’ refers to the Status field under the ‘Welcome Packet’ section of a referred student’s page. This section is visible if a student has been referred and has a welcome packet associated with their referral. This section might not be visible in the case of extremely old referrals that did not have a concept of welcome packets (and would need a new referral created).

Please note that the Onboarding Forms link takes about 2 minutes to appear after a referral has been submitted.

Engineering Review

Upon initial creation of the referral and welcome packet, the welcome packet may be in this status as our system processes data. Depending on how many other referrals and welcome packets are being processed at the same time, the welcome packet may be in this status for a few seconds and we would not expect you to see this often, if at all.

Requested

Once automations and data checks are completed, the welcome packet moves into 'Requested' status.

In Progress

If any of the onboarding form components are begun but not completed in full, it will move to this status. It will also remain in this status if responses to the intake questionnaire require additional communication and discussion.

Completed

Once onboarding forms are provided and verified, welcome packet moves to 'Completed' status

Canceled

This will occur in edge cases of no longer needing, accidentally having requested additional consents, and other special circumstances. This should occur rarely.
