[DRAFT] Create a Form

Learn how to create a form in TeamUp using the new multi-step builder, including requirements, timing, fields, and a shareable link.

šŸ“ Overview

As explained in our here, forms are used to collect data from your customers using Customer Fields.

Before creating a new form, we recommend reviewing the pre-loaded forms that the system automatically creates, to check what customer data is already being collected. Click here to see the pre-loaded forms, it might be that you simply want to edit an existing form instead by adding or removing fields.


šŸ› ļø Setup

To create a new form, head to Forms and Documents >> Customer Forms and click Add a Form:

1ļøāƒ£ Name and resubmission

Enter the Name of your new form. Note that the wording used will be displayed in the customer purchase or registration flow. Then choose whether you'd like customers to resubmit the form again in the future, and if so, after how many weeks. For example, you may have health related questions you want customers to answer every 24 weeks.

2ļøāƒ£ Requirements

Choose what the form is required for. Requirements are organized by event type, meaning classes, courses, appointments, and rentals or court bookings, and separately by membership category. Within each one, you can decide whether the form is required for:

  • All registrations/memberships: all customers registering for any type of event and/or purchasing any membership will be prompted to complete the form.

  • No registrations/memberships: customers won't be prompted to complete the form regardless of what they register for or buy. Tip: select this option for both registrations and memberships if you're not ready for customers to complete the form, or you want to temporarily pause an existing form.

  • Only selected ones: choose exactly which registration types and/or membership purchases should prompt the form.

  • All except those selected: the form is required for everything except the specific event types and/or memberships you select. Note: using "All Except" makes the form required for all other types, including ones that don't exist yet.

šŸ’” The registration options are based on your Class Types. If you're looking to require the form for one specific session only, such as your 6AM classes, you'll need to set that up as its own Class Type.

3ļøāƒ£ Timing

Choose when this form is completed relative to the registration or purchase: Before (the customer must complete it first), After (they complete it afterward), or Customize. Choosing Customize dynamically lists every event and membership you selected in the Requirements step, so you can choose Before or After individually for each one.

4ļøāƒ£ Fields

Choose which Fields you want to include in your form. Click Add next to each one, and you can drag fields to reorder them.

If you want to create a new field that isn't currently in your Customer Field list, click Create a Field. This now opens as a modal without leaving the form builder.

Use the Search function if you've got a long list of Customer Fields to look through:

Preview the form to see exactly what your customers will see before it goes live.

6ļøāƒ£ Done

Your form is now active and will be included in the registration and/or membership purchase flow according to the settings you selected.
