Program Goal: Help manage the highest risk patients with Food boxes/MTM based on need
Target Population:
Aetna Medicaid Insurance (IL ONLY)
Diabetes Type 2 OR congestive heart failure OR chronic kidney disease (stage 3-5)
Completed HR Screening AND Annual Wellness Exam with Provider prior to referral
