Type: Medicaid- Food as Medicine

Program Goal: Help manage the highest risk patients with Food boxes/MTM based on need

Target Population:

  • Aetna Medicaid Insurance (IL ONLY)

  • Diabetes Type 2 OR congestive heart failure OR chronic kidney disease (stage 3-5)

  • Completed HR Screening AND Annual Wellness Exam with Provider prior to referral

RD Requirements:

Medallion Credentialing Application
Copies of CDR, license in home state and where pt resides*, CV
