Program Goal: To enhance patients' overall health and lessen food insecurity. We aim to provide education, Fresh Funds, meals, and community support. To ensure the efficacy of this plan, it is vital that patients attend follow-up appointments.

Target Population: ~25,000 eligible members 11.5k or 45% are (0-17 years old).

As of 2/10/26 any new members (who had never provided their payment method to Foodsmart before 2/10/260) will receive the following FS bucks.

Initial visit= $0

2nd Visit= $50

3rd Visit and all following = $25

Areas of interest for the purpose of this partnership are:

  • Member follow-up visit rate (we want them coming back!)

  • NutriQuiz takes and retakes

  • Clinical outcomes (weight loss, blood pressure, cholesterol, HbA1c, etc.) – notated in FFD and the NutriQuiz.

  • Change in food security status

  • Teaching members how to use the Foodsmart Platform (meal planning, recipes, grocery shopping, etc)

  • NPS score

Depends on authorization from PCP (Max is up to 3 meals per day for up to six months).

Document patient health conditions and food allergies using this Google Form. This will allow the food order to be processed.
