Program Goal: Molina Health Maternal Health Pilot. The priority for Molina Health is helping their patients find, buy and eat healthy and affordable food for their families.

Target Population: 1,300 women who are pregnant OR have given birth within last 3 months

Members will receive 2-4 weeks of MTM depending on their coverage *

Within their 4 weeks, members may receive up to 10 meals per week, meant to provide 2 meals per day for five 5 days per week. 1st delivery of meals 1-2 week after completed visit.

*You do not need to complete a Risk Assessment for these members. A NQ is required at the 1st visit for members to receive food. The meals will start after the initial visit is closed.

All of these patients are pregnant or recently gave birth and are very excited about the four week meal benefit. These two Foodsmart Programs can be used as a starting point to create a customized program.

If patients are not participating in WIC you can complete the WIC PreScreening with them to see if they are eligible for benefits. WIC benefits include assistance to buy healthy foods like vegetables, fruits, beans and more.
