Program Goal: Improvements in food insecurity, lowering of A1C for patients with diabetes and healthy pregnancy.

Target Population: ~200 people who are maternal patients and/or patients with diabetes on medicaid. A mix of rural and urban populations with food insecurity

Patients will get Foodsmart Bucks to use through Instacart.

This program allows the following categories under Functional Foods:

  • Fresh and Frozen Fruit

  • Fresh and Frozen Vegetables

  • Seafood Including Fish

  • Dairy - yogurt only

  • Pantry - ONLY nuts, seeds, beans and nut butters
