Program Goal: The main goal is creating a meaningful reduction in food insecurity and delivering an incredible experience to receive a high NPS as well as overall goals of patients lowering their BMI.

Target Population: This is an expansion of the Elevance MO. This group of patients will likely see you for chronic condition management and also have one or more social determinants of health (SDOH) concern.

Must indicate patient current SNAP status on FFD (enrolled/applying/ etc). If a patient is not enrolled, help the patient apply for SNAP.

$25 after their first 2 visits. (FS Bucks will not reset in the new year)

Visits must be scheduled within 6 months of each other.

Patients will use Foodsmart Bucks through Instacart.

It is important that you close your charts within 12 hours of the visit start time. If charts are not closed the patient will not receive their Foodsmart Bucks.
