What is HRSN?

​​Health-Related Social Needs (HRSN) in healthcare refer to an individual’s unmet, adverse social and economic conditions—such as housing instability, food insecurity, or lack of transportation—that directly contribute to poor health outcomes and higher costs.

Some (not all) members will qualify for food support via HRSN services. The screening assessment for this will be done by our scheduling/support teams (not the RD). You are encouraged to remind the member on the visit that they will need to complete this questionnaire to be considered for food support. They will be contacted by foodsmart for the questionnaire or if the member self scheduled their visit, they can call Foodsmart to complete it.

Foodsmart is participating with the HRSN program to support identifying members' nutrition and food insecurity needs. Visits with Foodsmart RDs will support assessing member's eligibility to obtain medically tailored meals through the HRSN program.

The member must complete both the HRSN questionnaire and the RD visit to be submitted for consideration of food support.

The RD visit should be completed first, and the questionnaire to be completed after a visit is completed.

Food Vendor: Performance Kitchen

Food Length: If approved for food benefits, members will be eligible for up to 6 months of MTM, with specific length determined by the plan. To be submitted for consideration, members must complete an RD appointment AND the screening assessment done by our scheduling team.

Food Benefits Process:

  1. RD assessment note is submitted to the plan, along with a screening questionnaire done by our scheduling team

  2. The plan then makes determination on if member is eligible for food or not, and for what duration

  3. If approved, FoodOps team will reach out to member to confirm information and schedule food for delivery
