Select (Single Choice & Multi-Select)

  • For dropdown fields, begin typing to filter the available options. Matches will correspond to the available choices for the selected question.

  • To clear a selection, click the β€œX” next to the dropdown arrow.


Signature

  • On mobile devices, signatures are captured using your finger on the screen.

  • On desktop, use your mouse or trackpad to sign.

  • To redo a signature, click the Clear button and sign again.


Image

  • Add images by uploading from your device or using the device camera (available in the iOS and Android apps).

  • Add an optional caption or title (e.g., β€œScene showing slope conditions”).

  • Select the Remove button to delete an image.

  • Select the thumbnail to view a larger version or scroll through multiple images.


Text - Long

  • These questions are used in cases where significant written documentation is required, such as incident descriptions or documentation of first aid provided


Text - Short

  • These questions are designed for capturing brief free-form information, such as names, medications taken or known allergies.


Yes/No

  • These questions are designed for straightforward 'yes' or 'no' answers. For example, "Was the accident caused by a collision?".


Number

  • This question type only allows numeric values (e.g., '10' or '250') to be entered in the answer field.


Date Type

  • This question type uses a calendar picker to record the month, day, and year.


Date of Birth

  • This question type allows users to select the month, day, and year of birth for the individual. The system automatically calculates and displays their age once the date is complete.


Time

  • This question type captures time using the 24-hour clock format (4 digits). Time can be entered with or without the colon (e.g., 0742 for 7:42 AM or 1452 for 2:52 PM), as the colon is added automatically by the system.
