In order for our provider to submit to your insurance plan on your behalf, you need to have met the following requirements within a calendar month:

  • Onboarding call with your Lief coach in your first month

  • Wear your Lief device at least 16 days per calendar month

If you are working with a Lief coach, we’ll also submit your request for reimbursement for every 20 minutes of interaction and data review time. This time is automatically recorded by the Lief dashboard.

If you’ve met the above requirements, we will submit your request for reimbursement to your insurance plan at the end of each calendar month.

If you have met your deductible, depending on your health insurance plan, you will be billed for your copay. If you have not met your deductible and if you are subject to a co-insurance amount, you will be billed after your claim is processed by your insurance company. We always recommend confirming your co-pay and co-insurance amounts with your insurer.

If you have not met your deductible, you will be billed for the services you’ve utilized (see service price list here).

Claims processing time ranges from 14 days to 60 days, at which time you will receive an explanation of benefits (EOB) indicating what your insurance plan paid and what you owe, if applicable. Your EOB will typically arrive by mail, unless you've opted for electronic delivery.
