We believe in price transparency so that if you have in-network insurance with Lief, you can understand what we'll be billing your insurer, as well as prevent surprise bills.

If you have met your deductible, your plan should cover the full cost of services that you use. If you have not met your deductible, you will be responsible for paying for the Lief services you utilize until you meet that deductible. See the table below for a breakdown of those costs:

We highly recommend calling your health plan to check if the above services are covered under your plan (you can share the CPT codes listed above). At that time you can also gain a more specific understanding of any of your patient responsibility (e.g., deductible, co-pays, co-insurance, etc.).

Please note the wear time requirements (16 days per calendar month, any duration per day) in order for your insurance to cover your Lief rental. If you do not adhere to wearing your device for at least 16 days a month, you will still be charged for the $69 device rental cost that month. This $69 device rental fee will not be eligible for insurance reimbursement in that case, since you did not comply with the 16 days adherence requirement. We understand that new habits take time to form, thus this policy will begin applying to you from month 2 onward.

If you are uninsured or your health insurance plan is out-of-network, this information does not apply to you. Instead, you can access Lief as a consumer directly on our order page.
