Why Won’t My Medicare Details Validate?

What to do if your Medicare or Health Profile Verification is having an error or not validating.

Please ensure you fill out the 'reference number' field. If this is not filled out, the verify button will be grey and you won't be able to continue or verify.

Your Medicare details must exactly match what is recorded in the official Medicare database. Any mismatch in spelling, name format, or date of birth will cause a validation failure.

To ensure your details are correct:

  1. Visit my.gov.au.

  2. Log in and select Medicare from your linked services.

  3. Navigate to My details and click Update now.

  4. Confirm that:

    • Your full name matches exactly (including middle names if listed).

    • Your date of birth is accurate.

    • These match the information entered into your Hormn account.

Only the exact official record will validate successfully.
