When a breach record is logged, it can be automatically escalated to Supervisors, prompting them to investigate and determine the materiality of the breach.
Note: You must be a Supervisor for the Breach Reporting module to action the below steps.
To complete the verification task for a breach, follow the steps below:
Locate the record in the Task List on the Breach Reporting module homepage and click View.
Upon entering the record, click View on the task in the Task List.
Select the Breach Type from the multi-select options.
Select the Report Status and enter the rationale for the chosen status.
If remedial action is required, check the Remedial Action to be Taken box
Note: If this checkbox is selected, additional fields appear for you to enter the remedial action details, due date and assign this task to the relevant individual or group.
Click Complete Task.
Tip: Once you have completed the verification task, you can search for and tag the regulations breached against the record. This allows for reporting that can identify the most frequently breached outcomes and helps establish strategies for mitigating these types of breaches.
Add the outcome for a reported breach
If the breach is material and has been reported to the regulator(s), you can update the record to include the date the report was made and outcome of report, once know.
Within the breach record, click the Action drop-down and select Edit.
Update the Reported on field with the date the breach was reported to the regulator(s).
When known, enter the Outcome of Report.
Once all updates have been made, click Save.
