Why am I receiving the Google Forms Not Formatted Correctly Error?

If you are trying to export to Google Forms and you receive the error that the form is not formatted correctly, please check to make sure none of the questions have the same answer choices. You can click Edit to change the answer choices. Just ensure the correct answer choice doesn't change.
